If your spouse, domestic partner, child or other family member is enrolled in a UC-sponsored health plan (medical, dental or vision), you should have received a packet in the mail asking you to provide documentation that verifies their eligibility for coverage.
All faculty, staff and retirees who have one or more family members enrolled for coverage must provide the requested documentation. This project is the first step in a series of undertakings to ensure a more efficient administration of UC benefit plans.
Secova, Inc. is conducting the family member eligibility verification on behalf of UC. UC Human Resources is working with Secova to make the verification process as easy as possible.
May 17, 2012 is the deadline for submitting documentation. If you do not respond, you run the risk of having yourself and your family members de-enrolled from plan coverage.
If you submit documents such as tax returns or credit card statements, be sure to black out any personal information (social security numbers, financial data, etc.) not needed for the eligibility verification.
