If the PIE has expired, you must submit a statement of health for yourself – if you wish to purchase Supplemental Life – and/or for your spouse/same-sex domestic partner – if you wish to purchase Dependent Life insurance. The insurance company may or may not approve your enrollment. Children may be enrolled in Dependent Life insurance at any time without a statement of health.

If you are a newly appointed faculty member and don't enroll when first eligible, a second PIE starts on the first day of classes for the semester or quarter in which your appointment starts.

Generally, you cannot enroll in life insurance during Open Enrollment. You must do so during your PIE.

To enroll, see the Benefits Office or the person in your department who handles benefits.
