If your regular paid time increases, you may be eligible for increased benefits: Core Benefits if your appointments is at least 43.75% (17.5 hours per week). Mid-level Benefits if you are appointed to work at least 20 hours per week for at least one year or 100% for at least 3 months, but less than 12 months. Full Benefits if you work at least 20 hours per week for at least 12 consecutive months or if you accumulate 1,000 hours worked in the past 12 months. You will usually have a new 31-day PIE to enroll in any benefits for which you are newly eligible. If your regular paid time decreases, you must maintain an average regular paid time of at least 17.5 hours per week to remain eligible for your current benefit level. If your regular paid time falls below 17.5 hours per week, you become ineligible for medical (including Core), dental, vision, and Basic Life insurance. You also become ineligible for Short-term and Supplemental Disability coverage. You may still be eligible for Supplemental Life, AD&D, DepCare and/or Health Flexible Spending Accounts (FSA), Legal and Auto and Homeowners/Renters coverage. You may cancel your enrollment or continue in DepCare and/or the Health FSA to the end of the plan year. If your average regular paid time remains less than 17.5 hours per week, you will not be able to enroll for the following year. If you cancel enrollment, you may submit claims for eligible expenses incurred through the last day of participation in the plan(s). If you are a UCRP member, your membership continues. If your regular paid time decreases because of an approved Family and Medical Leave, your medical, dental and/or vision coverage will continue for up to 12 work weeks.
