All Health Care Reimbursement Account (HCRA) and/or Dependent Care Reimbursement Account (DepCare) account claims for services received for the 2005 plan year (January 1, 2005 through March 15, 2006) must be received by SHPS by June 15, 2006. Please note that the IRS requires that you forfeit any unclaimed money in your HCRA or DepCare account after the claim filing deadline.
You can download a claim form from the SHPS website or by calling SHPS directly at 1-877-270-3915 to request a form. Complete the form and fax it, along with documentation of your expenses, to 1-866-643-2219 or mail your form and documentation to the address indicated on the form.
